DHT (Dihydrotestosterone): What It Is & Levels
DHT, or dihydrotestosterone, is a powerful androgen hormone made from testosterone and plays an important role in sexual development, body hair growth, prostate function, and androgen-related hair loss. It is often discussed because of its connection to male pattern baldness, but DHT is not only a hair loss hormone.

What Is DHT?
DHT is a hormone in the androgen family. Androgens are often called male sex hormones, but they are present in both men and women. Testosterone is the best-known androgen, while DHT is a more potent derivative of testosterone in certain tissues.
DHT is important for male sexual development before birth and during puberty. It contributes to development of male external genitalia, facial hair, body hair, deepening of the voice, and other androgen-related changes. In adults, it continues to influence hair follicles, sebaceous glands, prostate tissue, and sexual characteristics.
Although DHT is often presented negatively because of hair loss, the hormone itself is not harmful by nature. Problems may occur when levels are unusually high, unusually low, or when certain tissues are genetically sensitive to its effects.
How Is DHT Made in the Body?
The body makes DHT by converting testosterone through the action of 5-alpha reductase. This enzyme exists in different forms and is active in tissues such as the scalp, skin, prostate, and reproductive organs.
This local conversion matters because DHT does not only circulate in the blood. It can also act inside tissues where it is produced. A person may not have extremely high blood DHT levels, yet the scalp follicles may still be highly sensitive to normal DHT activity.
This is one reason hair loss treatment can be complex. A blood test may not fully explain what is happening inside the scalp. In many cases, follicle sensitivity matters more than the total amount of DHT in the bloodstream.
Why Does DHT Matter for Hair Loss?
DHT matters for hair loss because it can contribute to follicle miniaturization in people with genetic sensitivity. In androgenetic alopecia, certain scalp follicles gradually become smaller under androgen influence. These follicles produce thinner hair shafts and spend less time in the growth phase.
In men, this process often affects the temples, hairline, mid-scalp, and crown. The back and sides of the scalp are usually more resistant, which is why they often remain thicker even in advanced male pattern baldness.
In women, DHT-related hair loss may appear as diffuse thinning over the top of the scalp, widening of the part, or reduced density rather than a sharply receding hairline. However, women with hair thinning should be evaluated carefully because iron deficiency, thyroid disease, hormonal changes, menopause, postpartum shedding, medication effects, and scalp inflammation can also contribute.
Does High DHT Always Cause Hair Loss?
High DHT does not always cause hair loss. Some people may have higher androgen activity but keep strong hair because their follicles are not highly sensitive. Others may have normal DHT levels but still develop androgenetic alopecia because their follicles react strongly to DHT.
This distinction is important. Hair loss is not simply a matter of too much DHT. It is often about how the follicles respond to DHT. Genetics determine much of this sensitivity.
That is why two brothers may have different hair loss patterns even if their hormone levels are similar. One may keep a full hairline into later adulthood, while the other may develop early recession or crown thinning.
What Are Normal DHT Levels?
DHT levels vary by age, sex, laboratory method, and individual biology. There is no single universal number that applies to everyone in every situation. A lab result should always be interpreted with the reference range provided by that laboratory and the patient’s symptoms.
In general, adult men have higher androgen levels than women. DHT levels also change across life stages. Levels are important during fetal development and puberty, while adult levels may be evaluated when symptoms suggest hormonal imbalance, androgen excess, or androgen deficiency.
A doctor usually does not diagnose a condition based on DHT alone. Testosterone, free testosterone, SHBG, DHEA-S, LH, FSH, prolactin, thyroid function, iron status, and other tests may be considered depending on the patient.
When Should DHT Levels Be Tested?
DHT testing may be considered when a clinician suspects an androgen-related issue, but it is not always the first test ordered. Many patients with hair loss do not need a DHT test to receive a diagnosis. Pattern, family history, scalp examination, and dermoscopic findings often provide enough information.
Testing may be more relevant in complex hormonal cases, unusual hair loss patterns, symptoms of androgen excess, delayed or abnormal puberty, certain reproductive concerns, or evaluation of conditions involving androgen metabolism.
Patients should not order hormone tests randomly and try to interpret them alone. Hormone results can be affected by timing, medication, lab method, health conditions, and biological variation. A normal result does not always mean there is no problem, and an abnormal result does not always identify the cause by itself.
What Can Cause High DHT?
High DHT may be related to increased testosterone conversion, genetic enzyme activity, androgen-producing conditions, certain medications, or hormonal imbalance. In men, DHT can be involved in prostate enlargement and androgen-related skin or hair changes. In women, high androgen activity may contribute to acne, oily skin, increased facial hair, scalp thinning, or menstrual irregularity depending on the underlying cause.
Polycystic ovary syndrome, adrenal disorders, ovarian hormone changes, or medication effects may be considered in women with signs of androgen excess. In men, high androgen activity should be interpreted in context because symptoms can overlap with other conditions.
High DHT should not be treated without understanding why it is high. Blocking DHT without a proper diagnosis may create unwanted effects or overlook the real cause.
What Are Symptoms of High DHT?
Symptoms of high DHT or increased androgen activity may include oily skin, acne, increased body or facial hair, scalp hair thinning, and changes related to prostate tissue in adult men. In women, signs may include unwanted facial hair, acne, irregular periods, and thinning hair over the top of the scalp.
However, symptoms do not always match blood levels neatly. A person with normal blood DHT may still experience androgenetic hair loss because the follicles are sensitive. Another person with elevated androgens may not lose hair if the follicles are less responsive.
Because symptoms overlap with many conditions, evaluation should include the whole clinical picture. Hair loss, acne, and oily skin can have hormonal, inflammatory, genetic, and lifestyle-related factors.
What Can Cause Low DHT?
Low DHT may occur because of reduced testosterone production, reduced 5-alpha reductase activity, certain medications that block DHT production, pituitary or testicular conditions, aging-related hormone changes, or rare genetic conditions.
In developing males, low DHT activity can affect sexual development. In adults, low androgen activity may contribute to reduced body hair, lower libido, sexual function changes, fatigue, reduced muscle mass, mood changes, or other symptoms, although these symptoms can have many causes.
DHT does not act alone. Low testosterone, thyroid disease, depression, sleep problems, anemia, medication effects, and chronic illness may create similar symptoms. This is why a broader medical evaluation is often needed.
Can DHT Be Too Low?
Yes, DHT can be too low in certain contexts, especially when androgen function is important for development or adult sexual health. However, low DHT is not always a problem by itself. It depends on symptoms, age, sex, testosterone levels, and medical history.
Some medications intentionally reduce DHT to treat conditions such as male pattern hair loss or prostate enlargement. In these cases, lower DHT is part of the treatment goal. Still, patients should understand possible side effects and discuss them with a doctor.
Hormones work in balance. The goal is not always to make DHT as low as possible. The goal is to manage the medical condition while protecting overall health and quality of life.
How Is DHT Connected to Male Pattern Baldness?
Male pattern baldness is strongly linked to DHT sensitivity in scalp follicles. The classic pattern includes temple recession, a thinning crown, and gradual loss across the top of the scalp. The sides and back usually stay stronger because these follicles are less sensitive to DHT.
The Norwood Scale is often used to describe male pattern hair loss stages. Early stages may show mild recession. Moderate stages involve deeper hairline changes and crown thinning. Advanced stages show larger bald areas on the top of the scalp.
DHT-related follicle miniaturization can begin years before obvious baldness appears. Early diagnosis can help patients protect existing hair before too many follicles become inactive.
How Is DHT Connected to Female Pattern Hair Loss?
Women also produce androgens, including testosterone and DHT, but at lower levels than men. Female pattern hair loss may involve androgen sensitivity, especially when thinning appears on the top of the scalp and the part becomes wider.
However, women require careful evaluation because many non-DHT factors can cause or worsen thinning. Iron deficiency, thyroid problems, hormonal transitions, childbirth, menopause, stress, autoimmune disease, restrictive diets, and medications can all affect hair density.
A woman with hair loss should not assume DHT is the only cause. A proper diagnosis can prevent ineffective treatment and identify correctable medical factors.
Do DHT Blockers Work?
DHT blockers can help some patients with androgenetic alopecia by reducing DHT activity or lowering DHT production. Finasteride and dutasteride are examples of prescription medications that affect 5-alpha reductase activity. They are commonly discussed for male pattern hair loss and certain prostate conditions.
These medications may slow hair loss and improve density in selected patients, but they are not suitable for everyone. Possible side effects, sexual function concerns, mood changes, fertility questions, pregnancy precautions, and medical history should be reviewed before use.
Patients should not start DHT blockers without medical guidance. A treatment that helps one person may not be appropriate for another.
What About Natural DHT Blockers?
Many supplements and natural products are promoted as DHT blockers. These may include saw palmetto, pumpkin seed oil, green tea extracts, nettle root, and other ingredients. Some people use them because they prefer non-prescription options.
However, natural does not automatically mean effective or risk-free. Supplements can vary in quality, interact with medications, and cause side effects. Evidence for many natural DHT blockers is less consistent than for prescription treatments.
Patients should discuss supplements with a clinician, especially if they take medication, have liver problems, have hormone-sensitive conditions, or are preparing for surgery.
Can Diet Change DHT Levels?
Diet may influence overall hormone health, weight, inflammation, insulin sensitivity, and nutrient status, but no simple diet can reliably control DHT in every person. Hair loss should not be reduced to one food group or one supplement.
A balanced diet with adequate protein, iron, zinc, vitamin D, healthy fats, and overall calories supports hair health. Severe dieting, rapid weight loss, and low protein intake can trigger shedding, even if DHT is not the main problem.
For patients with androgen-related symptoms, diet may be part of a broader plan, especially when insulin resistance, weight changes, or metabolic health are involved. Still, diet should support medical care rather than replace it.
Does Exercise Affect DHT?
Exercise can affect hormones, metabolism, stress, sleep, and body composition. Strength training and intense exercise may temporarily influence androgen levels, but normal exercise is not usually considered a direct cause of hair loss.
For most people, exercise supports overall health and may help manage stress, sleep quality, weight, and insulin sensitivity. These benefits can indirectly support hair and hormone balance.
Excessive training combined with poor nutrition, rapid weight loss, or inadequate recovery may contribute to shedding. The issue is usually overall body stress rather than exercise alone.
How Are DHT-Related Conditions Treated?
Treatment depends on the condition. For androgenetic alopecia, options may include minoxidil, DHT-lowering medications, low-level laser therapy, platelet-rich plasma in selected cases, and hair transplant surgery when appropriate.
For prostate-related conditions, DHT-lowering medications may be used under medical supervision. For women with androgen excess, treatment may involve hormonal evaluation, management of underlying conditions, and targeted therapy based on diagnosis.
The same DHT-related pathway can produce different symptoms in different people. Treatment should be personalized rather than based on a single hormone number.
Should Hair Transplant Patients Care About DHT?
Yes, hair transplant patients should understand DHT because native hair may continue to thin after surgery. Transplanted hair taken from resistant donor areas often holds up better, but existing hair around it can keep miniaturizing if androgenetic alopecia continues.
A good hair transplant plan considers DHT sensitivity, future hair loss risk, donor capacity, age, and maintenance treatment. Without long-term management, a patient may need additional procedures later as native hair continues to thin.
DHT control may be discussed before or after hair transplantation, depending on the patient. The decision should be made with medical guidance.
What Questions Should Patients Ask?
Patients concerned about DHT should ask clear questions. Is my hair loss pattern related to androgenetic alopecia? Do I need hormone testing? Are my symptoms caused by DHT or another condition? Would DHT-lowering medication help me? What are the risks? Are there safer alternatives? How long before results can be judged? Should I combine treatment with minoxidil or other approaches? How will progress be monitored?
Good answers should be specific to the patient. A responsible clinician will not blame every symptom on DHT or promise a guaranteed result from one treatment.
